The role centers on assessing needs and coordinating services to enhance life quality. Schedule is full-time, 8:30 AM–4:30 PM, Monday–Friday, with no telework option, and residency in PA is required.

Qualifications

• Six months as Aging Care Manager 1 or County Caseworker 1; or completion of County Social Casework Intern program; or six months of professional human services or nursing experience with a bachelor’s degree in social sciences or related field; or an equivalent combination of experience and training.

As an Aging Care Manager 2 with the Northumberland County Area Agency on Aging (AAA), you help older adults overcome challenges and stay safe in their communities. This role centers on assessing needs and coordinating services that enhance the quality of life for older adults. As an Aging Care Manager 2, you will perform the following duties:

• Needs Assessment: Evaluate social contacts, daily living activities, cognitive functioning, physical health, and available resources
• Service Coordination: Connect older adults with agencies, hospitals, clinics, courts, and community organizations that support their needs
• Comprehensive Reviews: Conduct detailed assessments of medical, social, psychological, and housing conditions to support effective care planning
• Interagency Collaboration: Work with professional staff and community partners to discuss diagnoses and plans of treatment
• Benefits Guidance: Assist older adults in managing income, accessing Social Security and Medicare, and understanding legislation that affects benefits
• Case Documentation: Prepare social studies, case histories, reports, and records

Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
• Telework:You will not have the option to telework in this position.

QUALIFICATIONS

Minimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1;or
• Successful completion of the County Social Casework Intern program;or
• Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.
