Qualifications

• Six months as an Aging Care Manager 1 or County Caseworker 1 or completed County Social Casework Intern program.
• Bachelor's degree in social sciences, behavioral sciences, human services, or related field.

Responsibilities

• Develop informed care plans based on medical, social, psychological, and functional assessments.
• Ensure legal and regulatory time frames are met while following policies and procedures.
• Conduct investigations into abuse, exploitation, abandonment, or self-neglect.
• Complete referrals, arrange services, and monitor follow-up to maintain quality.
