Job summary

The Lebanon County Area Agency on Aging is seeking an Aging Care Manager 2 who coordinates programs to enrich the lives of older adults and support vulnerable populations. This role focuses on protective services, investigations, and care plan coordination to enhance safety and well-being.

Responsibilities include receiving reports, developing plans, ensuring regulatory compliance, collaborating with agencies, maintaining SAMS records, and providing educational outreach as needed.

Qualifications

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

Responsibilities

• Protective Service Reports: Receive and investigate reports of need for protective services for older adults
• Care Plan Development: Assess needs and coordinate service plans that support safety and reduce risks
• Regulatory Compliance: Follow state regulations and required procedures after obtaining certification
• Interagency Coordination: Work with police departments, social service staff, and health care providers to support protective service cases
• Data Management: Enter and maintain consumer information in the SAMS system, including assessments and service histories
• Community Outreach: Provide educational seminars to care providers when requested
