Job summary

Rocky Mountain Care Group is seeking an LPN through RMCare Talent Solutions in Murray, UT for in-person care across skilled nursing, home health, and hospice settings. You will provide direct patient care, administer medications, monitor residents, assist with assessments, and maintain EMR documentation.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• Current CPR certification.
• Experience in long-term care and/or home health and hospice preferred.

Responsibilities

• Provide direct patient care and monitor resident conditions.
• Administer medications and treatments as ordered.
• Monitor vital signs and report changes in condition.
• Assist with assessments and care planning.
• Document care accurately in electronic medical records.
• Collaborate with physicians, RNs, and interdisciplinary care teams.
