Job summary

Gentiva in Lubbock, Texas seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will conduct assessments, visits, and admissions, and maintain clinical oversight for assigned patients.

You will collaborate with physicians and the IDG, manage care plans, document accurately, and support compliance and training while promoting compassion and teamwork.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year RN experience in acute care, oncology, hospice or home health.

Responsibilities

• Provide direct in-person care during after-hours, weekends, and on-call shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Coordinate with IDG, update plan of care, and communicate changes promptly.
