Behavioral Health Network, Inc (BHN) is recruiting an Aftercare Coordinator to support Hampden County Sheriff’s Office clients. You will conduct discharge planning for Medication Assisted Treatment, ensuring smooth transitions from jail to community care, with strong community ties and data-entry responsibilities.

The role requires a background in SUD settings, bilingual English/Spanish, and the ability to coordinate with outpatient providers and MOUD options to maintain continuity of care.

Qualifications

• High School Diploma or GED plus 2-3 years’ relevant experience in an SUD or related co-occurring position.
• Associate’s Degree plus 1-2 years’ experience or Bachelor’s Degree in related field.
• CHW Certification or Recovery Coach Certification is a plus.
• 2 years’ experience helping individuals obtain resources and navigate systems in SUD/behavioral health.

Responsibilities

• Discharge planning related to MAT in the community and maintaining continuity of care.
• Meet with assigned program individuals within specified timeframes and coordinate with court and treatment timelines.
• Enter session content accurately into HCSO data systems.
• Connect individuals with outpatient services and MOUD information for community placement.
• Arrange enrollment in OTP/OBAT at preferred community providers.

What You Will Do

• The Aftercare Coordinator at the Hampden County Sheriff’s Office (HCSO) is responsible for discharge planning as it relates to Medication Assisted Treatment (MAT) in the community. The Aftercare Coordinator at HCSO has diverse knowledge of biopsychosocial, spiritual, emotional, and aftercare planning dimensions of substance use disorder. The Aftercare Coordinator maintains well-established community relationships to help ensure a smooth transition and continuity of care for those individuals served who are re-entering the community.
• Meets with assigned program individual served in accordance with standards set forth by HCSO (identified below):
• Pre Trial
• Within 2 business days of referral
• Within 5 days prior to Court Date
• Within 5 days after Court Date
• Otherwise, every 30 days
• Sentenced
• Within 5 days from referral
• Every 30 days
• Once 60 days to potential release, every 2 weeks
• Within 5 days Prior to Release
• Section 35 Civil Commitments
• Within 5 days from referral
• Every 2 weeks
• Responsible for accurate and timely entry of session content into all appropriate HCSO data systems.
• Informs program individuals served how to contact staff should they be released prior to formal admission to a community clinic.
• Obtains appropriate release(s) of information to facilitate the process of ensuring continuity of care in the community.
• Meets with the individual served within 2 business days of referral to connect the individual served with general outpatient services in the community, as well as MOUD community information (applicable for individuals who qualify for and request Medication for Opioid Use Disorder (MOUD) but will be released before they are able to be enrolled).
• Arranges for the individual served to be enrolled in a community-based OTP/OBAT of their preference to ensure that the treatment started will continue or facilitate the return of individual served to their previous OTP or community provider (applicable for planned discharges to the community).

Who You Are

• High School Diploma or GED plus 2-3 years’ relevant experience in an SUD or related co-occurring position required or Associate’s Degree in related field with 1-2 years’ experience, or Bachelor’s Degree in a related field.
• Community Health Worker (CHW) Certification or Recovery Coach Certification additional plus.
• 2 years’ experience helping to obtain resources and/or navigate systems to individuals with SUD and behavioral health problems.
• Bilingual (English/Spanish)
• Must meet contractor criteria required by HCSO that may include extensive background investigation, drug testing, and medical clearance.
• Must attend and successfully completes HCSO onsite orientation.
• Valid driver’s license, reliable vehicle for all working hours, and valid automobile insurance required.

PAY: $21.00 - $26.00 (depending upon experience)
