Mountainside Treatment Center is hiring an Aftercare Coordinator to join our Continuing Care team in Canaan, CT. The role emphasizes organizing and delivering ongoing care plans, coordinating with clinicians, families, and referral sources, and maintaining comprehensive documentation.

The position requires attention to detail, strong communication, and the ability to manage a caseload of up to 15 clients while complying with regulatory standards.

Qualifications

• High school diploma with 2 years in human services/addiction treatment or related field preferred
• Bachelor's degree (BS/BA/BSSW) from an accredited college preferred
• Proficiency with Microsoft Office suite, Adobe, internet apps, and CRM/EMR systems

Responsibilities

• Develops and provides continuing care plan/resources prior to discharge
• Collaborates with client’s treatment team to develop an aftercare plan
• Communicates with family, providers and referral sources with weekly status updates
• Manages a caseload of up to 15 clients
• Maintains notes in CRM/EMR and completes daily scheduled tasks
• Complies with regulatory requirements
