Job summary

Qualifications

• Bachelor's degree, preferably in Human Services or related field.
• Minimum of two years professional experience in mental health counseling.
• Pass criminal background clearance and HIPAA/privacy standards.

Responsibilities

• Interact and communicate professionally with clients, staff, and outside contacts.
• Maintain client confidentiality per agency policies.
• Attend and participate in staff meetings, supervisory conferences, and trainings.
• Orient and assess all aftercare clients via approved tools.
• Develop service plans with client input and monitor progress.
• Schedule and facilitate case management meetings with clients as required.
• Identify resources and link clients to needed services.
