Job summary

TFI Family Services, Inc. is seeking a full-time Aftercare Case Manager in Parsons, Kansas to support children and families navigating permanency through reintegration, adoption, or aging out of care.

You will coordinate services, document cases, provide crisis intervention, and connect families with community resources while receiving training to grow professionally.

Qualifications

• Bachelor’s degree in human services
• Valid driver’s license required
• Willingness to learn—training provided

• Support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.
• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.

• Meet with families in-person to support reunification and resolve any concerns.

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:
