Job summary

TFI Family Services is seeking an Aftercare Case Manager in Kansas to support families who have achieved permanency through reintegration, adoption, guardianship, or aging out of care. You will focus on risk management, crisis intervention, and coordinating community services.

The role requires a bachelor’s degree in human services and a valid driver’s license, with training provided to grow in the position. This full-time role offers competitive hourly pay and a mission-driven team.

A Day in the Life

As An Aftercare Case Manager, You’ll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. You’ll

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
