Job summary

2INgage is seeking an Aftercare Case Manager in Independence, KS. You will support children and families achieving permanency through reintegration, adoption, guardianship, or aging out of care, with a focus on stability and crisis intervention.

You’ll collaborate with agencies, maintain documentation, and coordinate community services to support reunification. A Bachelor’s degree in human services and a valid license are required, plus training will be provided.

A Day in the Life

As An Aftercare Case Manager, You’ll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. You’ll

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
