Parkland Health is seeking an Aftercare Coordinator at New Parkland Hospital in Dallas, TX. The role operates on the day shift (8:00 am–4:30 pm), Monday–Friday, with full-time employment.

The position requires an RN license or equivalent Texas licensure with relevant direct patient care experience and involves coordinating post-discharge services and patient education. Responsibilities include developing complex aftercare plans, coordinating referrals, and ensuring timely documentation.

Qualifications

• RN license in good standing or valid temporary permit with Texas Board of Nursing (or equivalent license)
• Licensure in Physical Therapy, Occupational Therapy, Speech-Language Pathology, or Social Work per Texas requirements
• 3+ years Direct Patient Care experience (preferred)
• Ability to coordinate aftercare services and communicate across care teams

Responsabilités

• Assesses complex patient aftercare needs and barriers to care transitions.
• Manages aftercare programs, workflows, caseload priorities, and quality-improvement activities.
• Develops individualized aftercare plans addressing follow-up treatment, meds, appointments, and community resources.
• Coordinates referrals, authorizations, appointments, and provider communication.
• Documents outcomes, interventions, and barriers within established timeframes.
• Consults with patients, families, clinicians, payers, and external providers to coordinate services.
