Job Details

The After Hours Hospice RN Supervisor is responsible for the supervision of both the RNs (Triage & Field) and the CSRs who are emergently triaging the calls received from patients and/or caretakers for Hospice & Palliative Care program

This working supervisory role provides supervision of the staff that patient physical and psychosocial health systems assessments telephonically, supporting the Hospice Plan of Care and “Goals of Care” based on the patient’s and caretaker’s needs/wishes, and facilitates coordination with plan developed/placed by the primary RN Case Manager for the patient.
