Job summary

Gentiva Hospice is seeking an After Hours/On-Call RN to provide compassionate, high-quality care in patients’ homes during evenings, nights, weekends, and holidays. You’ll act as the primary liaison among patients, families, physicians, and the hospice team, ensuring timely symptom management and continuity of care.

Requirements include an active RN license in the employment state, at least 1 year of nursing experience, CPR certification, and a valid driver’s license.

Qualifications

• Current RN license in the state of employment
• 1+ years of nursing experience
• Hospice heart: compassionate, calm under pressure
• Excellent clinical judgment and emotional resilience
• Comfort providing care in patients' homes or care facilities

Responsibilities

• Report to Administrator/Executive Director or Patient Care Manager Senior
• Provide direct patient care after normal business hours
• Serve as primary liaison between patients, families, physicians, and hospice team
• Respond promptly to urgent calls, visits, and symptom management
• Evaluate patient and family needs and guide care decisions
• Collaborate with LPNs/LVNs, CNAs, and other staff
• Document clinical visits and communications accurately
• Participate in quality assessment and performance improvement
• Work independently while coordinating with daytime team
