Job summary

Gentiva Hospice in the United States seeks an RN for after-hours, on-call hospice care. You will work weekends and respond to patient needs in evenings, nights, and holidays, delivering compassionate care in home or facility settings.

This role reports to the Administrator or Patient Care Manager and collaborates with an interdisciplinary team to manage symptoms and plan care while ensuring accurate documentation.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year of nursing experience.
• Hospice heart: compassionate, calm under pressure.
• Valid CPR certification.

Responsibilities

• Provide direct patient care after hours and weekends.
• Serve as liaison between patients, families, physicians and hospice team.
• Respond promptly to urgent calls and symptom management needs.
• Evaluate patient and family needs to guide care decisions.
• Collaborate with LPNs/LVNs, CNAs, and support staff.
• Document visits and communications accurately.
• Participate in quality assessment and performance improvement programs.
• Work independently while coordinating with daytime team.
