Qualifications

• Current RN license to practice in Georgia.
• Current CPR certification.
• 1–2 years of home care experience; hospice experience preferred.
• Good verbal and written communication and problem-solving skills.

Responsibilities

• Complete an initial patient and family assessment to determine hospice care needs.
• Provide a comprehensive physical assessment and history of illness.
• Use assessment data to determine nursing diagnoses.
• Triage calls and assist patient/family with problem resolution; counsel on nursing needs.
• Make home visits for symptom management and support.
• Modify treatment plans based on patient responses and conditions.
• Maintain accurate, detailed reports per policy.
• Monitor and report symptoms to the IDG.
• Provide health care instructions per assessment and plan.
• Obtain physician orders as required.
• Pronouncement of patients.
• Other duties as assigned.
