Job summary

Longleaf Hospice and Palliative Care in Georgia seeks a Transitional Care Nurse to provide direct nursing care, care coordination and patient education in general inpatient units.

The role involves patient assessment, plan of care, discharge planning and collaboration with hospice physicians to meet patient and caregiver needs. 7 ON / 7 OFF shift schedule (7:00 AM–7:00 PM) every other week. Experience in hospice or home care preferred; CHPN preferred.

Qualifications

• Licensed as a registered nurse in the state of Georgia.
• Graduate from an accredited school of nursing.
• Minimum of five years nursing experience.
• Minimum of two years' hospice/home care experience preferred.
• CHPN certification preferred.
• Strong communication and problem-solving skills.

Responsibilities

• Provides direct nursing care and care coordination for hospice inpatients.
• Educates patients and families about home hospice and palliative care.
• Leads and participates in IDG meetings and discharge planning.
• Monitors and communicates patient condition and progress.
• Acts as primary coordinator across disciplines for patient care.
