Job summary

Seattle's Union Gospel Mission is seeking an After-Care Case Manager II to work with a team dedicated to a safe, healing environment within a Christian transitional living program. You will collaborate with internal and external partners to coordinate transitional and permanent housing, case management, mental health, and spiritual counseling.

Qualifications

• Associate Degree is required; combination with four years of relatable experience may be substituted.
• Three years in social services or rescue mission; one year in a Case Manager role.
• Proficient in Microsoft Office Suite.
• Possess a valid Washington State Driver’s License with clean driving record.

Responsibilities

• Assist with training new staff and provide training to team members.
• Collaborate with program ministries to implement After-care in a timely manner.
• Develop housing stability plans with guests and outside housing resources.
• Facilitate community events for guests and alumni.
• Work with a Relationship Manager to partner with social services for transitional living.
• Follow up on referrals and maintain relationships with partner organizations.
• Manage a caseload of guests in a transitional after-care program.
• Create and maintain guest files and prepare program reports on outcomes.
