Job summary

Kennedy-Donovan Center is seeking a Case Manager for Adult Foster Care in Cape Cod and Islands, including West Yarmouth and New Bedford sites. The role focuses on coordinating home- and community-based support for adults with developmental needs, ensuring person-centered planning and timely documentation.

The position offers full-time hours (40 per week), a Monday–Friday daytime schedule, and competitive benefits.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Nursing, Public Administration or related field.
• Strong communication and organizational abilities with interpersonal skills.
• Proficiency with Electronic Health Records (EHR) and Microsoft Office Suite.

Responsibilities

• Coordinate AFC home placements and support services per individuals’ needs and goals.
• Facilitate referral, intake, assessment, and placement processes.
• Develop, implement, and review ICPs/ISPs with multidisciplinary teams.
• Collaborate with individuals, families, and providers for coordinated care.
• Maintain accurate documentation and update electronic health records in systems like iCentrix.
