Job summary

Helix Human Services is seeking a Case Manager in Springfield, MA, to provide intensive case management for youth in the Comprehensive Foster Care program. The role focuses on stabilizing homes, coordinating services, and ensuring continuity of care.

The position is full-time with hybrid work options. A Bachelor's degree and 3+ years of experience, plus license-eligibility and a driver’s license, are required.

Qualifications

• Bachelor's degree with a minimum of three years relevant experience.
• Licensed social worker or license eligible.
• Valid driver’s license.
• Knowledge of Hampden County.

Responsibilities

• Provide intensive case management to children and youth referred by the Department of Children and Families (DCF) to the Comprehensive Foster Care (CFC) program, with the goal of long-term stability and permanency.
• Case Manager is responsible for providing intensive case management supports to children and adolescent youth assigned to CFC foster care homes including day-to-day functioning of the home, advocacy, and crisis intervention.
• Visit each assigned foster child a minimum of one time weekly.

Salary Range: $50,000.00 To $70,000.00 Hourly

Case Manager

Provide intensive case management to children and youth referred by the Department of Children and Families (DCF) to the Comprehensive Foster care program (CFC), with the goal of long-term stability and permanency. Case Manager is responsible for providing intensive case management supports to children and adolescent youth assigned to CFC foster care homes including day-to-day functioning of the home, advocacy, and crisis intervention. Participate in the screening of potential youth and preparing for their placement in the CFC program, including contact in advance with all collaterals, preparation of placement paperwork, and referrals to health /therapeutic providers in advance of placement to provide for continuity of services. Visit each assigned foster child a minimum of one time weekly.

Helix Human Services provides Comprehensive Foster Care (CFC), a unique program through which foster families provide a home to one or two children and/or adolescents in need of more support.

Qualifications
