NLACRC in Santa Clarita Valley is seeking a Consumer Services Coordinator to provide information, advocacy and service arrangements for individuals and families. The role includes developing and monitoring program plans, coordinating services, documenting forms, and advocating with community agencies.

Formación

• Bachelor’s degree in psychology, social work, sociology, or related human services field with at least two years of related experience.
• Master’s degree may substitute for experience.
• Bilingual Spanish may be required depending upon position.

Responsabilidades

• Provide information, advocacy, and service arrangement for consumers and families.
• Participate in development of program plans for individuals; monitor and evaluate and revise as needed.
• Identify and coordinate services for individuals and their families.
• Documentation, completing forms and reports in accordance with regulations and NLACRC policies.
• Serve as an advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals served, family members, service providers, advocates, etc.
• Identify barriers to service delivery and implement culturally sensitive strategies with empathy, trust, and respect; work in diverse settings.
