Job summary

Creative Works in Maine is seeking a Community Case Manager to support adults with intellectual disabilities and autism in Southern Maine and the Lewiston/Auburn area. This full-time role involves coordinating services, writing plans, and advocating for participants to access meaningful supports.

Qualifications

• Bachelor’s degree in psychology/behavioral health/social work or related field.
• Or a degree in an unrelated field plus at least 1 year of relevant human services experience.

Responsibilities

• Routinely communicate with service recipients, families and guardians.
• Develops, writes, and updates service plans, reports, and correspondence.
• Organizes and facilitates Person Centered Planning and service review meetings.
• Collects and compiles relevant case history information.
• Conduct intake and orientation for new participants to Community Case Management services.
• Support and monitor individual access and maintenance of benefits and entitlements.
• Develop and update Person Centered Plans and ensure ongoing implementation and entry into the EIS system.
• Submit all progress notes, reviews, reports in a timely manner.
• Enter billing for contact hours weekly.
• Act as primary liaison between CW and participants' family, social worker, state agencies, and staff for Targeted Case Management.
