Job summary

HealthPartners is seeking a registered nurse to provide high quality patient care and ensure effective nursing processes and care coordination. The role emphasizes comprehensive assessments, developing care plans, delegating tasks, and educating patients and families within a multidisciplinary team.

The position requires MN RN licensure, Wisconsin RN eligibility within two months, and current BLS certification.

Qualifications

• Graduate of an accredited school of nursing
• Current RN licensure in Minnesota
• Current RN licensure in Wisconsin within two months of hire
• Current BLS certification for health professionals required and maintained (online BLS not acceptable)
• Strongly demonstrated written and oral communication skills
• Preferred: One year full time RN experience

Responsibilities

• Provides high quality service to both internal and external customers.
• Provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition.
• Delegates nursing tasks when appropriate and supervises/monitors function.
• Creates and implements dynamic teaching plans based on patient need.
• Acts as a resource for other staff.
• Nursing Process: assessment, problem identification, planning, implementation, evaluation.
• Care Coordination: supports patients and families, educates, identifies services.
• Education: partners with patient/family, teaches self-care, participates in professional education.
• Leadership: models nursing practice, demonstrates flexibility, supports staff, communicates effectively.
