Health Partners Plans seeks a skilled ambulatory care nurse to deliver patient- and family-centered care in clinics. You will collaborate with providers and support staff to implement nursing interventions while honoring cultural beliefs and patient priorities. A current RN license is required.

You will participate in team huddles, follow care protocols, document in EMR, and contribute to quality improvement and patient safety. Strong communication and ongoing professional growth are essential.

Formación

• Specialized Diploma graduate of an accredited school of nursing required.
• Knowledge of how to convey respect and collaborate with patients and designated care partners in care and planning.
• Cultural awareness and sensitivity across diverse backgrounds.
• Ability to communicate effectively with patients, families, and care teams.
• Understanding of effective communication concepts, tools, and techniques.

Responsabilidades

• Integrates Evidence-Based Practice with clinical expertise, patient and family preferences, and cultural beliefs for optimal care.
• Collaborates with clinic care providers to ensure timely communication between primary care and specialists.
• Supports implementation of clinical care pathways and participates in quality improvement opportunities.
• Utilizes ambulatory care nursing interventions during visits, calls, and electronic communication.
• Provides education to patients and families on preventive care, chronic disease management, and medications.
• Documents all interactions and nursing care in EMR according to standards and regulations.
• Performs care coordination for patients, including abnormal results, triage, vaccinations, and forms.
• Oversees clinical care essential to health maintenance and wellness.
