Job summary

Palm Medical Centers is seeking a Primary Care Nurse Practitioner to join our premier team in Florida. This role emphasizes value-based primary care with a focus on patient quality of life and reduced burnout.

You will work in an in-person, private-practice setting with strong compensation, excellent benefits, and a 8am–5pm, Monday–Friday schedule. Relocation assistance and career development opportunities are available.

Qualifications

• 2–3 years of experience in primary care/value-based care settings with Medicare Advantage, Medicare, and Medicaid populations.
• Active Florida APRN license required; DEA license or eligibility within 2 months of hire.
• Strong documentation and patient relationship-building skills; understanding of value-based care metrics.

Responsibilities

• Deliver high-quality primary care to adult patients in a value-based model.
• Collaborate with a multidisciplinary team to optimize care plans.
• Document thoroughly in the EMR and monitor chronic conditions.

Job Requirements:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Florida APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
