HealthyU Clinics in Phoenix, AZ is seeking an Advanced Practice Provider (NP/PA) to lead the care team and deliver comprehensive primary care. You will provide medical oversight, leadership, and patient-centered services to a panel of patients.

Responsibilities include evaluations, diagnoses, treatment plans, collaboration with physicians, quality improvement, and accurate EHR documentation. Must have NP/PA training, state licensure, BLS, DEA, and at least one year in primary care.

Qualifications

• Completion of an accredited NP or PA program.
• Active and unrestricted state NP/PA license (required by first day).
• Current BLS certification.
• Active DEA registration.
• Ability to provide two professional references.
• Minimum of one year of clinical experience in a primary care setting.
• Experience providing longitudinal patient care and continuity of care.
• Knowledge of managed care and quality programs.

Description

As an Advanced Practice Provider (APP) – Nurse Practitioner/Physician Assistant’s play a key role in leading the care team and delivering comprehensive primary healthcare to a panel of patients. You will provide medical oversight, expertise, and leadership to ensure the delivery of high-quality, cost-effective healthcare services. Key responsibilities include:

• Provide patient-centered, comprehensive primary care services consistent with a value-based care model
• Conduct patient evaluations, diagnoses, and treatment plans based on evidence-based guidelines
• Work collaboratively with physicians and other healthcare professionals to ensure continuity of care
• Participate in quality improvement programs and contribute to the evaluation of practice performance metrics
• Ensure compliance with privacy regulations, quality programs, and all HealthyU Family Medicine policies, procedures, and protocols
• Utilize and implement new healthcare technologies, products, services, and programs to enhance patient care
• Educate patients on preventive care, chronic disease management, and overall wellness
• Maintain accurate and up-to-date patient documentation in the Electronic Health Record (EHR) system
• Perform other duties as assigned

Requirements

• Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
• Active and unrestricted state license as a Nurse Practitioner or Physician Assistant (required by the first day of employment)
• Current Basic Life Support (BLS) certification
• Active DEA registration
• Ability to provide two professional references
• Minimum of one year of clinical experience in a primary care setting
• Experience providing longitudinal patient care and continuity of care
• Knowledge of managed care principles, including quality assurance, quality improvement, risk management, population health, and disease management programs
• Demonstrated ability to collaborate effectively within a multidisciplinary healthcare team
• Excellent interpersonal and communication skills with patients, providers, and staff
• Self-motivated with a patient-centered, results-driven approach to care
• Bilingual in English and Spanish strongly preferred
