Job summary

HealthyU Clinics is seeking an Advanced Practice Provider (NP/PA) to deliver comprehensive primary care under physician supervision. You will lead care teams, oversee medical oversight, and ensure high-quality, cost-effective services for a patient panel.

The role requires NP/PA licensure, BLS, and experience in primary care. Travel up to 50% with a valley-wide Arizona scope.

Qualifications

  • Completion of an accredited NP or PA program.
  • Active NP or PA license by start date.
  • Current BLS certification.
  • One+ years of clinical experience in primary care.
  • Experience with longitudinal patient care and managed care systems.
  • Ability to work in a multidisciplinary team.
  • Excellent communication with patients and colleagues.
  • Spanish language proficiency preferred.

Responsibilities

  • Provide patient-centered, comprehensive primary care services within a value-based care model.
  • Conduct patient evaluations, diagnoses, and treatment plans based on evidence-based guidelines.
  • Collaborate with physicians and other healthcare professionals to ensure continuity of care.

Position Description:

The APP Pain Management will provide health care services to patients under the direction of a physician. APP's are academically and clinically trained to perform many of the routine but time-consuming tasks that physicians usually do and are accountable for their own actions and accountable to their supervising physicians.

As an Advanced Practice Provider (APP) – Nurse Practitioner/Physician Assistant’s play a key role in leading the care team and delivering comprehensive primary healthcare to a panel of patients. You will provide medical oversight, expertise, and leadership to ensure the delivery of high-quality, cost-effective healthcare services.

Key Responsibilities:
  • Provide patient-centered, comprehensive primary care services consistent with a value-based care model
  • Conduct patient evaluations, diagnoses, and treatment plans based on evidence-based guidelines
  • Work collaboratively with physicians and other healthcare professionals to ensure continuity of care
  • Participate in quality improvement programs and contribute to the evaluation of practice performance metrics
  • Ensure compliance with privacy regulations, quality programs, and all HealthyU Family Medicine policies, procedures, and protocols
  • Utilize and implement new healthcare technologies, products, services, and programs to enhance patient care
  • Educate patients on preventive care, chronic disease management, and overall wellness
  • Maintain accurate and up-to-date patient documentation in the Electronic Health Record (EHR) system
  • Perform other duties as assigned
Required Qualifications:
  • Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
  • Valid state-issued license as an NP or PA (must be active by the first date of employment)
  • Current BLS (Basic Life Support) certification required.
  • Ability to provide two professional references
  • One or more years of clinical experience in primary care
  • Experience providing longitudinal patient care (continuity of care)
  • Familiarity with managed healthcare systems, including quality assurance, quality improvement, risk management, population health, and disease management programs
  • Strong ability to work collaboratively within a multidisciplinary team
  • Excellent communication skills with both patients and colleagues
  • Self-motivated and results-driven approach to patient care
Preferred Requirements:
  • Bilingual (Spanish) strongly preferred
  • Experience with Athena EMR
