Job summary

HealthyU Clinics seeks a Primary Care Provider to lead a collaborative care team and deliver comprehensive, patient-centered care for an established panel. Our practice maintains a lower daily patient volume of about 15–18, enabling thorough evaluations and individualized treatment plans.

You will diagnose, manage chronic diseases, coordinate referrals, and document accurately in the EMR while adhering to regulatory standards.

Qualifications

• MD or DO degree in Family Medicine or Internal Medicine
• Active, unrestricted MA state medical license (must be active by start date)
• Current BLS certification
• ActiveDEA registration
• Minimum of2 years of clinical experiencein a primary care setting
• Experience providing longitudinal, patient-centered care
• Familiarity with value-based care, population health, and quality improvement initiatives
• Strong clinical judgment and decision-making skills
• Excellent communication and interpersonal skills
• Ability to work effectively within a team-based care environment

Responsibilities

• Deliver comprehensive, patient-centered primary care aligned with HealthyU’s value based care model and commitment to improving patient outcomes.
• Conduct routine health assessments, diagnose and treat acute conditions, and provide ongoing management of chronic diseases.
• Lead and collaborate with a multidisciplinary care team to ensure coordinated, efficient, and high-quality care delivery.
• Participate in quality improvement initiatives and monitor key clinical and performance metrics to support positive patient outcomes.
• Provide preventive care, health education, wellness counseling, and population health management.
• Develop individualized care plans based on patients’ clinical needs, health goals, and overall well-being.
• Coordinate referrals, follow-up care, and communication with specialists and other healthcare providers to promote continuity of care.
• Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR).
• Adhere to all applicable clinical, regulatory, compliance, and organizational standards, including HealthyU policies and Code of Conduct.
• Support the implementation and adoption of new care models, technologies, workflows, and clinical programs that enhance patient care and practice efficiency.
• Perform minor office based procedures and respond appropriately to urgent clinical needs within the scope of practice.
• Educate patients regarding diagnoses, treatment options, medications, preventive health, and healthy lifestyle practices.
• Stay current with evidence-based clinical guidelines, best practices, and continuing medical education requirements.
• Promote a culture of compassionate, accessible, high-quality care that prioritizes meaningful patient relationships and improved health outcomes.
