Job summary

Theoria Medical is seeking a full-time Advanced Practice Provider to serve residents in Wyoming, MI skilled nursing facilities. You will provide high-quality, patient-centered care with daily rounding, medication management, and acute issue response, while using a charting-focused iPad and collaborative clinical team.

Qualifications

• Active U.S. license and board certification required.
• Specialty expertise in Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatrics or related fields.
• Experience in skilled nursing facility settings preferred.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds, prescribe medications and treatment plans.
• Communicate with facility staff via secure messaging or phone.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards and facility policies.
