The role emphasizes coordination of care, adherence to established protocols, and active participation in quality improvement and community resource linking. Travel to headquarters may be required occasionally.

Qualifications

• Certification as a Family Nurse Practitioner or Physician Assistant.
• Current valid state license to practice.
• Compliance with licensure requirements and quality programs.

Responsibilities

• Provide primary care to patients under supervision of a physician and following established medical protocols.
• Collaborates with physician preceptor to formulate and review medical protocols.
• Assist with development of operational policies and procedures.
• Assist medical director in supervising other clinic staff.
• Obtain patient history, identify risk factors, and provide health education.
• Perform physical exams, obtain labs, and analyze clinical data for diagnosis and treatment.
• Ensure treatment plan aligns with protocols and adjust as needed.
• Document all pertinent information accurately and timely.
• Obtain signed consent forms and ensure proper transfer of records when referred.
• Identify community resources to support patient needs.
• Consider cultural and economic factors in care planning.
• Assist in policy development and quality improvement activities.
• May coordinate Centering Group Visit Programs.
• Participate in ongoing quality improvement initiatives.
• Assist in daily lab, imaging, and record review.
• Manage registries as assigned.
