Job summary

• Prescribe medications in accordance with laws, regulations, and policies.
• Provide preventive care, health education, and chronic disease management as appropriate.
• Maintain accurate patient records and clinical documentation.
• Coordinate care with physicians, nursing staff, behavioral health, and the multidisciplinary team.
• Support quality improvement, patient safety, and organizational clinical initiatives.
