Job summary

Sentara Community Care is seeking a compassionate Family Nurse Practitioner to join our mobile care team serving Hampton Roads and Northeast North Carolina communities. This part-time role (20 hours/0.5 FTE) works from a Mobile Care RV to provide primary care for children and adults in underserved areas, collaborating with a multidisciplinary team.

The ideal candidate has 1+ year APP experience, experience in FQHC/community health, and a commitment to holistic patient care.

Qualifications

• 1+ year APP experience required.
• Experience in FQHC or Community Health Center is beneficial.
• Ability to co-manage patients with a diverse care team.

Responsibilities

• Provide primary care via Mobile Care RV to children and adults in underserved communities.
• Treat family medicine, pediatrics, and adult patients with complex issues.
• Collaborate with RN, Counselors, Care Management and other team members.
• Review quality metrics monthly with the healthcare team.

Position Highlights

• Locations: Hampton Roads VA and Northeast North Carolina
• Part-Time: 20 hours/ 0.5 FTE
• Working from Mobile Care RV providing direct primary care services to children and adults in underserved communities
• Treat family medicine, pediatrics, and adults with complex issues
• Shared call schedule, with RN first call
• Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients' health holistically.
• Review quality and outcome metrics monthly with the healthcare team
• Collaborate with local civic and faith-based leaders on community education programs
• Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
• 1+ year APP experience required

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
