Job summary

Our team provides essential primary care services through a network of community clinics and mobile health units, caring for uninsured and Medicaid populations. Position Highlights:

Location: Harrisonburg and Broadway, VA

Schedule : Monday - Friday 9:30-6:30p. 1-2 Saturdays per month

Working from Mobile Care RV providing direct primary care services to children and adults in underserved communities at designated locations such as Hope Distributed (Food Pantry & Clothing Ministry) and Navigation Center (Homeless Services Center).

Partner with City Schools providing sports physicals, immunizations, etc.

Shared call schedule, with RN first call

Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients' health holistically.

Review quality and outcome metrics monthly with the healthcare team

Collaborate with local civic and faith-based leaders on community education programs

Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice

1+ year APP experience required

The ability to speak Spanish is extremely helpful in this role, but not required

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
