Job summary

SSM Health DePaul Hospital in Bridgeton, MO seeks a PRN Nurse Practitioner or Physician Assistant to join the Complex Care Clinic. This outpatient role focuses on pre-surgical optimization and post-discharge care to reduce readmissions.

You will collaborate with primary care and specialty teams, perform pre-op assessments, coordinate care, and provide timely referrals. A Master's degree and MO licensure/PA certification are required, with at least two years in Internal Medicine or Adult Primary

Qualifications

• Master's degree as a Nurse Practitioner or Physician Assistant.
• Minimum of 2 years of current experience in Internal Medicine or Adult Primary Care.
• Missouri NP licensure or Physician Assistant certification/licensure.
• Strong assessment, clinical judgment, and patient management skills.

• Collaborate closely with primary care providers and specialists to coordinate patient care.

This unique outpatient role focuses on helping patients optimize their health before surgery and supporting successful transitions following hospital discharge, ultimately improving outcomes and reducing readmissions.
