Job summary

SSM Health DePaul Hospital in Bridgeton, MO is seeking an experienced Physician Assistant for a part-time Complex Care Clinic role. The position focuses on pre-surgery optimization and post-discharge care to improve outcomes and reduce readmissions, in an outpatient setting.

The role requires strong evaluation skills and collaboration with PCPs and specialists. The schedule is variable (0.5 FTE) with clinic hours M-F, 7:30a-4:30p, and no evenings or weekends.

Qualifications

• Master’s degree in Physician Assistant required.
• Minimum two years of current PA experience in internal medicine.

Responsibilities

• Perform pre-surgery evaluations and determine optimization needs per clinical pathways.
• Manage transitions post-discharge to reduce readmissions.
• Collaborate with PCPs and specialists; coordinate testing and medication where appropriate.

SSM Health DePaul Hospital in St. Louis, MO, is recruiting for an experienced and part-time (0.5 FTE) Physician Assistant for our Complex Care Clinic at SSM Health DePaul Hospital (Bridgeton, MO) & SSM Health St. Joseph West Hospital (Lake St. Louis, MO).

This unique outpatient role focuses on helping patients optimize their health before surgery and supporting successful transitions following hospital discharge, ultimately improving outcomes and reducing readmissions.

Job Highlights:

• Outpatient only with variable weekly schedule (alternating 3 days per week and 2 days per week)
• Clinic is open M-F from 730a-430p
• No evenings/weekends/call
• Complete pre surgery evaluations and determine optimization needs based on algorithms and clinical pathways
• We focus on optimizing medical conditions and often coordinate care with patients PCP or specialiststhere is limited ordering of testing and medications with this portion of the role
• The transitional care is a post discharge clinic focused on preventing hospital readmissions. We evaluate patients within the first week post discharge and assess status, and treat/refer as needed
• This role requires strong evaluation and assessment skills
• Limited diagnostic workups and prescriptions involved in this setting
• Strong department onboarding and mentorship
• Fantastic team and department culture and collaboration

Requirements:

• Master's degree in Physician Assistant
• Minimum of two (2) years' current experience working as an PA in internal medicine
• Missouri certified physician assistant
