Job summary

SSM Health is seeking a Nurse Practitioner or Physician Assistant for a PRN Complex Care Clinic role spanning SSM Health DePaul Hospital (Bridgeton, MO) and SSM Health St. Joseph West Hospital (Lake St. Louis, MO).

This outpatient position emphasizes pre-surgical optimization, care coordination, and post-discharge follow-up to improve outcomes and reduce readmissions. Responsibilities include pre-surgical evaluations, collaboration with PCPs and specialists, and timely post-discharge assessments

Qualifications

• Master's degree as a Nurse Practitioner or Physician Assistant.
• Missouri NP licensure or Physician Assistant certification/licensure.
• Minimum of 2 years of current experience in Internal Medicine or Adult Primary Care.

Responsibilities

• Conduct pre-surgical evaluations and determine optimization needs using clinical pathways.
• Collaborate with primary care providers and specialists to coordinate patient care.
• Evaluate patients after discharge and provide timely assessment, treatment or referrals.
• Focus on clinical evaluation and care coordination with limited diagnostic workups and prescribing responsibilities.

What You'll Do:
