Theoria Medical seeks a full-time Advanced Practice Provider to support skilled nursing facilities in a post-acute care setting. You will deliver patient-centered care, round daily, and develop treatment plans in coordination with a collaborative clinical team.

Qualifications

• Active U.S. license and board certification for NP (ANCC, AANPCB, PNCB) or PA (NCCPA).
• Specialties include Internal/Family/EM or Geriatric medicine and related fields.
• Ability to work in post-acute/SNF settings with flexible scheduling.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding and develop treatment plans.
• Prescribe medications and address acute issues.
• Coordinate with facility staff via secure messaging/phone.
• Maintain reliable on-site attendance and ensure regulatory compliance.
