Theoria Medical is seeking a full-time Advanced Practice Provider to serve local skilled nursing facilities. The role emphasizes relationship-based care, autonomy, and a supportive team using intuitive technology.

Formación

• Active U.S. license and board certification in NP or PA.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, geriatrics.
• Proficiency with EHRs and clinical decision support tools.

Responsabilidades

• Provide patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds and develop treatment plans.
• Coordinate with facility staff via secure messaging.
• Ensure regulatory compliance and proper charting.
