Job summary

Oak Street Health in Topeka, KS is hiring a Nurse Practitioner to deliver value-based primary care to adults on Medicare in our outpatient clinics. You will work on a collaborative care team and see patients independently when appropriate, focusing on preventive care, chronic disease management and wellness.

The role reports to the Center Medical Director, involves visits, wellness exams, care coordination, and patient education, with a Monday–Friday schedule and full-time hours.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
