Could you help transform the way care is delivered for people living with frailty in Dundee?

The Nutrition and Dietetic Service in Tayside is seeking to recruit an experienced Advanced Practice Dietitian to work within the Frailty at Home Service across the Dundee Health & Social Care Partnership area.

The Frailty at Home Service supports people living with frailty in their own homes and community settings, helping to prevent avoidable hospital admissions, reduce deconditioning and support independence. Working closely with acute hospital teams, the service promotes seamless transitions of care and ensures people receive the right care, in the right place, at the right time.

The postholder will be an integral member of the multidisciplinary Frailty at Home Team, providing specialist dietetic assessment and intervention for people with complex nutritional needs. The role requires a proactive approach to clinical care, alongside contributing to service improvement, pathway development and the ongoing development of this innovative model of care.

Working across organisational boundaries, the successful candidate will provide specialist dietetic input within community and hospital settings as required. This will include in-reach to acute services and outreach into the community to support integrated frailty pathways, timely discharge, admission avoidance and continuity of nutritional care.

Alongside advanced clinical practice, the postholder will influence the future delivery of frailty services, lead service evaluation and quality improvement activity, helping to demonstrate service impact and build the evidence base required to sustain and further develop this model of care.

Applicants should have significant post-registration experience as a specialist dietitian, including managing complex nutritional care across a range of settings. Experience in service development, quality improvement, audit or service evaluation is highly desirable. Experience of collecting, analysing and presenting outcome data to support service improvement and future service planning would be advantageous.
