Qualifications

• Registered Dietitian with HCPC
• Significant post-registration clinical experience

• Lead service evaluation and quality improvement activities
• Develop and implement frailty pathways across community and hospital settings
• Engage in in-reach to acute services and outreach to the community for integrated care
