• APC role requires advanced clinical knowledge and the ability to perform histories and physical exams for diverse patient populations.
• Strong documentation practices and adherence to clinical pathways and timely progress notes are essential.
• Ability to collaborate with physicians, nurses, and interdisciplinary teams to coordinate care.

Responsibilities

• Assess health status and perform comprehensive H&P for acute and complex patients.
• Order and interpret diagnostic tests and manage treatment plans.
• Coordinate daily clinic schedules with the attending physician team and discharge planning.
• Prescribe diagnostic strategies and therapeutics to achieve outcomes.
• Document progress notes, discharge summaries, and care plans in the medical record.
