Theoria Medical is seeking a full-time Advanced Practice Provider in Battle Creek, MI to serve local skilled nursing facilities. You will care for residents over time, prescribing treatments, performing rounds, and coordinating with facility staff to ensure high-quality care.

Qualifications

• Active U.S. license and board certification as an NP or PA (ANCC/AANPCB/PNCB/AACN for NPs or NCCPA for PAs).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.
