Qualifications

• Licensed Registered Nurse and Nurse Practitioner in the state, with certification in adult, family, or gerontology health.
• Master's degree in Nursing from an accredited program.
• Bilingual skills may be required based on operational needs.

Responsibilities

• Administers full scope of advanced nursing practice for patient populations, including assessments and interventions.
• Leads interdisciplinary care teams and participates in rounds to manage patient care.
• Performs post-discharge visits, assessments, and urgent interventions including medication management and patient education.
• Collaborates with patients, families, and healthcare providers to develop and implement care plans.
• Ensures compliance with care standards, manages documentation and billing, and maintains credentials.
• Provides care virtually via telehealth or in patients' homes, adapting to various environments.
• Acts as a peer reviewer and preceptor, participating in quality improvement initiatives.
• Assumes responsibility for ongoing professional development and special projects.

• Collaborates with patients, families, and healthcare providers to develop and implement care plans.
• Ensures compliance with care standards, manages documentation and billing, and maintains professional credentials.
• Provides care virtually, via telehealth, or in patients' homes, adapting to various environments.
• Acts as a peer reviewer, preceptor for students, and participates in quality improvement initiatives.

Qualifications
