Job summary

Live Well Homecare is seeking a Clinical Nurse Practitioner to provide advanced nursing leadership in home-based care. This role focuses on delivering high-quality patient care, evaluating responses, and developing protocols under the supervision of the Clinical Director.

You will lead inter-professional teams, train staff, and ensure care plans align with state and national standards as well as quality measures such as HEDIS and QARR.

• Minimum two years of NP experience preferred, with clinical home care or managerial experience advantageous
• Knowledge of coding, quality measures, and bilingual skills as needed
