Harris Health is looking for an Advanced Practice Nurse, Hospital at Home (APN-HaH) in Houston, Texas. The APN-HaH will provide comprehensive patient care and be responsible for planning, implementing, and communicating the patient’s care plan.
The ideal candidate will have a Master’s degree in Nursing, current certifications, and experience in a hospital setting. Preference will be given to those with bilingual skills in Spanish.
The Advanced Practice Nurse, Hospital at Home (APN‑HaH) is accountable for providing comprehensive patient care congruent with the role and approved population (Family/Individuals across lifespan and Adult/Gerontology) through systemic assessment, diagnosis, treatment, management, and follow‑up affecting direct patient care under the supervision of the HaH Clinical Medical Director. The APN‑HaH serves as a clinical expert and consultant to support the healthcare system in providing patient‑centered, culturally‑competent, outcome‑focused therapeutic care within the management of specified populations through independent and collaborative clinical management. The APN‑HaH aligns practice with professional rules and regulations, accreditation standards, and organizational policies and procedures.
This APN‑HaH provides high‑quality patient care to various patient populations within the framework of the Hospital at Home Program model. The APN‑HaH will be accountable for planning, implementing, evaluating, and communicating all phases of the patient’s plan of care to the medical and nursing teams. This APN is responsible for facilitating an environment that supports professional practice and empowers all interdisciplinary health‑care workers to provide safe, effective, compassionate, and efficient patient care.
