Qualifications

Responsibilities

The role involves collaborating with patients, families, and healthcare providers to develop and adjust individualized care plans, ensuring adherence to regulatory standards and quality measures such as Patient-Centered Medical Home, HEDIS, and QARR. It includes participating in interdisciplinary team meetings, guiding team performance, and ensuring documentation and billing compliance. The nurse practitioner may perform procedures within collaborative agreements and provide peer reviews, precept students, and contribute to clinical projects. For psychiatric roles, additional duties include psychosocial assessments, mental health treatment, crisis intervention, and collaboration with psychiatrists. The role requires maintaining licensure, certifications, and ongoing professional development.
