Responsibilities

• Manage 14–16 follow-up patients daily in an outpatient cardiology setting.
• Perform chart reviews and in-basket management.
• Deliver evidence-based care and patient education.
• Collaborate with a supportive interdisciplinary team.
• Maintain accurate documentation in the EMR and compliant coding/billing.
• Participate in quality, safety, and performance initiatives.
