Prime Healthcare Illinois Medical Group is seeking a Nurse Practitioner or Physician Assistant to support our cardiology team. This role offers outpatient and limited inpatient care to patients with cardiovascular conditions in a high‑acuity setting, working with board‑certified cardiologists.

Qualifications

• Current Nurse Practitioner or Physician Assistant license in Illinois.
• Cardiology experience.
• At least 1 year of experience in cardiology or internal medicine preferred.
• ACLS and BLS certification required.

Responsabilités

• Conduct patient assessments and cardiac evaluations.
• Manage chronic cardiac conditions such as CHF, CAD, arrhythmias, and post‑MI care.
• Assist with medication management, lifestyle counseling, and patient education.
• Collaborate with cardiologists to monitor and adjust treatment plans.
• Support post‑discharge follow‑up and transitional care.
