Qualifications

Responsibilities

• Coordinate with patients and care teams to ensure timely treatment.
• Manage follow-up care, consults, and tests in the care plan.
• Process patient secure messages via MyHealthyVet (MHV) and coordinate with the team.
• Assist in team huddles, set priorities, and adjust workflows for patient needs.
• Enter and verify patient data in the electronic record and assess readiness for visits.
• Schedule, reschedule, or cancel appointments using VSE; communicate with providers.
